The article focuses on side effects from blood products. Currently, the residual risk for bacterial transmissions in platelet concentrates and severe septic diseases consists of 1:1,428 and approximately 1:50,000, respectively. While some countries have implemented bacterial blood donor screening programs, the shelf-life of platelet concentrates was reduced from 5 days to 4 days in Germany. With the introduction of real-time PCR methods the risk of transfusion transmitted infections such as HCV could be reduced to 1:11 million. The risk of TRALI is currently 1:65,000 and 1:2.26 million for plasma products and red cell concentrates, respectively, which is comparable to the residual risk for serious bacterial contaminations.

Ethischer Kodex für Blutspende und Bluttransfusion
Zusammenfassung
The blood donation is a traditional task of the Red Cross, which has been performed by the national Red Cross and Red Crescent societies in co-operation with their respective national governments all over the world already since the twenties. Apart from the fi rst aid the blood donation is worldwide one of the most known Red Cross tasks. The Red Cross makes about 1/3 of the world’s blood demand available. In numerous States the Red Cross has accomplished breakthrough achievement in the fi eld of transfusion medicine and haematology. Thereby the activities range from the recruitment of donors and the care of the donor to the professional production of high quality and safe blood products. The German Red Cross also identifi es itself with the blood donation as a humanitarian duty orientated on the general welfare to the public. The blood donation service has been part of the listed tasks in conformity with the statutes of the German Red Cross on all levels of the association since 1952. The Red Cross confesses itself to the principle of the voluntary and non-remunerated blood donation. This contribution makes a plea towards an upkeeping of this principle and therefore to avoid a change in the character of the blood donation in Germany.
This review presents a risk-adapted treatment concept based on absolute risks of thrombosis and bleeding. Hereditary and acquired thrombophilic risk determinants increase the risk of fi rst and recurrent venous thromboembolism (VTE) and absolute risks associated with thrombophilic risk factors in comparison with absolute risks of severe bleeding allow an individual risk-benefi t. In case the VTE was provoked by a reversible risk factor such as surgery or oral contraceptives, the risk of recurrence is low and three months of oral anticoagulation appears to be adequate. Indications for an indefi nite oral anticoagulation are less well defi ned in international guidelines. In favor of an indefi nite oral anticoagulation (1-3% bleeding risk per year) are two or more idiopathic events, antiphospholipid syndrome, malignancy, and combined or severe thrombophilic defects like antithrombin defi ciency or homozygous factor V Leiden in patients with a fi rst idiopathic VTE (risk factors increase thrombosis risk from 4% to ≥8% per year).
There was not paid very much attention to transmissible spongiform encephalopathy (TSE) in transfusion medicine for a long time. Results of animal studies and fi rst cases of vCJD, which are probably transmitted by transfusion, brought TSE into the focus. Despite interrupting of the alimentary prion infection chain transfusion transmission of vCJD is still possible. There is a more or less effective removal of prions during production processes of plasma derivatives. Removal of prions by fi ltration of labile blood components seems to be successful but it is not yet realized under routine conditions. Although the possibilities to identify donors at risk are limited, the probability of transfusion-transmitted vCJD is extremely small in Germany.
