Die sichere Versorgung mit Blut bei Großschadensereignissen stellt große Herausforderungen an die versorgenden Blutspendedienste. Im Folgenden werden zu erwartende Bedarfe, aber auch die Voraussetzungen für die gesicherte Versorgung und Verteilung dargestellt. Eine sichere Versorgung ist in Deutschland möglich. Sie ist aber nur dann zu gewährleisten, wenn unabhängig von aktuellen Ereignissen stets eine ausreichende Zahl von Blutspendern zur Blutspende kommt, die es den Blutspendediensten ermöglicht, einen Vorrat von ca. drei Tagesbedarfen sicher zu stellen. Darüber hinaus ist zur Sicherstellung der Versorgung im Krisenfall eine stärkere Einbindung der Blutspendedienste in die Strukturen der Krisenkommunikation und Organisation anzustreben.
A save and adequate blood component supply is essential to treat casualties in mass casualty events and at the same time it is a great challenge for blood donation services. We analysed the anticipated demand of blood components from the literature as well as the requirements for ensuring adequate stock levels of blood components and distribution logistics. Save and adequate stock levels of blood components can only be guaranteed with steady blood donations from committed volunteer donors and an organisation that ensures stock levels sufficient for three days. Furthermore, a stronger integration of blood donation services into the regional disaster planning and organisation is required.

Die Rolle der Blutgruppen bei der Versorgungssicherung mit Blutkonserven
Zusammenfassung
Engpässe in der Versorgung der Krankenhäuser mit Erythrozytenkonzentraten (EK) der Blutgruppe 0 RhD negativ treten immer wieder auf. Die Autoren diskutieren Ursachen und mögliche Lösungsvorschläge, um der Verknappung dieser EK vorzubeugen. Neben den Blutspendediensten können auch die Krankenhäuser durch vorbeugendes Management dazu beitragen, dass die für die Notfallbehandlung wichtigen EK der Blutgruppe 0 in ausreichender Menge vorhanden sind und der Verfall an EK der Blutgruppen B und AB reduziert wird. Eine enge und vertrauensvolle Zusammenarbeit zwischen transfundierenden Ärzten, Mitarbeitern der Blutdepots und des versorgenden Blutspendedienstes führt zu einer konsequenten Umsetzung der Richtlinie Hämotherapie und dem verantwortungsvollen Einsatz der wertvollen Ressource Blut.
In Germany shortages in the blood supply of hospitals occur repeatedly, especially for the packed red blood cell concentrates (RBC) of blood group 0 RhD negative. The authors discuss potential reasons and ways to solve this problem. Both, the blood transfusion services and hospitals, are able to help saving these important resources for emergency hemotherapy and to reduce the waste of RBC blood group B and AB by prophylactic measures and preventive management. A close collaboration between medical doctors, colleagues in charge of the blood banks and the supplying blood transfusion service result in a successful implementation of the applicable guidelines and a responsible use of the valuable resource blood.
Die maschinelle Autotransfusion
Zusammenfassung
Die maschinelle Autotransfusion – das Auffangen, Waschen und Retransfundieren von Wundblut – ist eine effektive Maßnahme, um den Fremdblutbedarf bei größeren Blutverlusten von > 500ml zu reduzieren. Dadurch müssen weniger Patienten den potentiellen Risiken einer Fremdbluttransfusion ausgesetzt werden, was der Patientensicherheit zugutekommt. Bei Patienten mit komplexen Antikörpern oder seltener Blutgruppe, aber auch bei solchen, die allogene Blutprodukte kategorisch, etwa aus religiösen Gründen, ablehnen, sollte die maschinelle Autotransfusion (MAT) immer als eine bluterhaltende Maßnahme Anwendung finden. Vor der Retransfusion müssen Kontraindikationen wie bakterielle oder infektiöse Kontaminationen sicher ausgeschlossen werden. Bei Eingriffen in der Herz- und Gefäßchirurgie sowie in der Orthopädie findet die MAT bereits regelhaft Anwendung. Auch in der Tumorchirurgie und der Geburtshilfe wird nach zusätzlicher Vorbehandlung des Wundblutes bereits teilweise mit autologem Blut gearbeitet, wobei der Einsatz in diesen Fachbereichen noch Gegenstand aktueller wissenschaftlicher Diskussionen ist.
Cell salvage – collecting, washing and re-transfusing blood lost during surgery – is an effective tool to minimize the need for allogenic red blood cells in the setting of blood loss. Thereby, the number of patients exposed to potential risks of allogenic blood transfusions is reduced and patient safety is increased. For patients presenting with complex antibodies or rare blood types, or for those who refuse transfusion of allogenic RBC for religious beliefs, cell salvage represents an important blood conserving method. Before re-transfusing of salvaged blood, contraindications such as bacterial or infectious contaminations must be excluded. Cell salvaging is commonly used during cardiac and vascular surgery as well as in orthopaedics, whereas its application (with additional cleaning steps) in obstetrics and cancer surgery has been tested but is still discussed by the scientific community.
Erythrozytenkonzentrate aus Stammzellen
Zusammenfassung
Die Vision, aus Stammzellkulturen eine Blutversorgung mit Erythrozytenkonzentraten zu ermöglichen, rückt in den Bereich der Machbarkeit. In Kultur erzeugte Erythrozyten könnten z. B. helfen, Versorgungslücken bei Pandemien zu schließen und die Infektionssicherheit zu vergrößern. Auch enthalten sie, anders als konventionelle EK, ausschließlich junge Erythrozyten. Mögliche weitere Qualitätsverbesserungen liegen bei chronisch transfundierten Patienten mit anti-erythrozytären Antikörpern, die z. B. mit autologen expandierten EK versorgt werden könnten. Auch birgt die gezielte Expression bestimmter Hämoglobine (z. B. Hämoglobin F) neue Möglichkeiten in der Optimierung der Sauerstoffversorgung für bestimmte Patienten. Eine große Herausforderung stellt derzeit noch die technische Lösung der Expansion im großen Massstab im Bioreaktor dar.
The vision of a blood supply by culture-expanded red cells (RBCs) is moving closer to reality. Culture-expanded blood cells might help to overcome risks in blood supply in cases of pandemia, increase infection safety, and could help to improve the situation in allo-sensitized chronically transfused patients. Also, expanded red cells are young, other than donated RBCs with an average age of 50–60 days. Expression of alternative hemoglobin forms such as HbF could be useful e. g. for intrauterine transfusions. The greatest challenge in the field is currently the adaptation of the expansion process to large scale production.

Momentan ist kein künstlicher Sauerstoffträger in Westeuropa zugelassen. Die prinzipielle Funktionalität verschiedener Präparate ist allerdings unbestritten; in Ländern wie z. B. Russland oder Mexiko stehen den Ärzten bereits zugelassene Präparate zur Verfügung. Aufgrund der vielversprechenden präklinischen sowie klinischen Pipeline besteht die Hoffnung, dass in absehbarer Zeit auch im westeuropäischen Raum künstliche Sauerstoffträger eine neue Behandlungsoption darstellen werden.
In Western Europe no artificial oxygen carrier has been approved yet. However, the general concept of artificial oxygen carriers is beyond dispute. In other countries such as Russia or Mexico, such substances are available for clinical use. The existence of promising preclinical and clinical pipelines yields hope that artificial oxygen carriers will provide an additional treatment in the near future in Western Europe, too.

Blutdepots in Krankenhäusern spielen bei der sicheren Versorgung von Patienten mit Blutkomponenten und Plasmaderivaten eine wichtige Rolle. Um die qualitätsgesicherte Führung von Blutdepots sicher zu stellen, sind eine Reihe von Anforderungen an die Beschaffung, Lagerung und Ausgabe der Arzneimittel zu berücksichtigen. Die entsprechenden Anforderungen werden in diversen regulatorischen Vorgaben adressiert. Interne Audits und die Aufsicht durch die Behörden sollen die Umsetzung sicherstellen und überprüfen.
Blood depots in hospitals play an important role in the secure supply of patients with blood products and plasma derivatives. To guarantee the quality of procurement, storage and dispensing of the blood products, various regulatory standards define rules. Internal audits and supervision by public authorities are tools to examine and promote the implementation of standards.
Arzneimittelversorgung von Krankenhäusern durch mehrere Apotheken unzulässig

Das Verwaltungsgericht Magdeburg hat festgestellt, dass die vertragliche Versorgung eines Krankenhauses mit Arzneimitteln durch mehrere Apotheken gemäß §§ 11 Abs. 3, 14 Abs. 4 und 5 S. 2 ApoG nicht zulässig sei. Zur Begründung wurde im Wesentlichen ausgeführt, dass die Versorgung eines Krankenhauses gleichsam „aus einer Hand“ erfolgen müsse. Dies gelte sowohl für die Regel- und Notfallversorgung mit Arzneimitteln als auch für die Regel- und Notfallberatung. Diese aktuelle Rechtsprechung ist auch für das Blutspendewesen nicht ohne Bedeutung. Der dem Apothekenrecht bei der Arzneimittelversorgung zu Grunde liegende Rechtsgedanke trifft auch auf die Versorgung von Krankenhäusern mit Blutprodukten zu, deren Gewinnung und Herstellung dem Transfusions- und dem Arzneimittelrecht unterworfen sind. Auch bei der Versorgung mit Blutprodukten findet eine Regelund Notfallversorgung sowie eine Regel- und Notfallberatung statt, deren Trennung nicht praktikabel erscheint, so dass auch hier die Versorgungssicherheit nur bei einer Blutversorgung „aus einer Hand“ gewährleistet sein sollte.
The Administrative Court of Magdeburg has stated that it is not admissible for a hospital to have a contractual supply of medicines by more than one pharmacy. The reasoning given mainly referred to the fact the supply of medicines to a hospital has to ensue – as it were – “from a single source”. This applies to the regular supply of medicines as well as to the emergency supply of medicines to a hospital and also to the adjoining regular consultation as well as to the consultation in cases of emergency. The present adjudication is also of relevance for blood donations due to the fact that a similar regulation valid for the supply of blood products to hospitals is not given in the enforced law governing blood donations. The judicial legal concept derived from the judgment and apparent to pharmacy law described above concerning the supply of medicines, also applies to the supply of blood products to hospitals. In these cases there is also a regular supply of blood products to hospitals and an emergency supply as well as the adjoining regular consultation and the emergency consultation for blood products in hospitals which cannot practicably be separated, so that a supply safety can only be guaranteed if the blood supply ensues “from a single source”.
