For most hospitals, massive transfusion is a rare, but occasionally appearing problem. The clinical definition is characterised by persistent transfusion requirements within a short time, haemodynamic instability and a problematic bleeding localisation. The treatment of bleeding is to stop the bleeding, aiming at a palpable radial pulse. Aetiologically, a coagulopathic (caused by a disturbance of the coagulation system) and a traumatic (caused by tissue injury) bleeding must be differentiated, while mixed forms are common. Standard laboratory parameters are used all over the word, while their effectiveness was never proven. Point of care diagnostics with viscoelastic tests and platelet functional analyses are quicker and more useful. A damage control resuscitation approach with permissive hypotension should be considered for every life-threatening bleeding. Essential are the preservation of preconditions, the application of tranexamic acid together with an individualised transfusion of blood products and coagulation factors. Within 24 h of stopping the bleeding, a thrombosis prophylaxis should be considered. An accurate documentation of transfused compounds is recommended.
During therapeutic hemapheresis, pathologic blood components are removed from the circulation and replaced by healthy donor cells or plasma if necessary. Therapeutic hemapheresis is used as first line therapy alone or in combination with drug therapy. Guidelines of the American Society for Apheresis using categories and recommendations of therapeutic hemapheresis are widely used. The kinetics of the pathological substance which has to be removed must be taken into consideration when planning therapeutic hemapheresis. The development of new drugs has to be taken into consideration in future indication of lipid apheresis.
Emerging production of advanced therapy medicinal product (ATMP) and innovative cellular therapeutics goes along with higher and urgent need for xengen-free culturing media. Quarantine free platelet lysate (PL) derived from expired thrombocyte concentrates provides a very good and human(e) alternative to (fetal) bovine serum (FBS or BS, respectively). We describe the production and further possible applications of PL.
Nowadays, molecular diagnostic methods are an important complementat of serologic diagnostic methods. In this manuscript, after an introductory survey on the molecular basis of antigens is given, the significance of molecular methods is exemplified for determination of antigens of platelets and neutrophils, analysis of serologic enigmas, analysis of weak D to determine the transfusion and prophylaxis strategy, prenatal antigen determination from maternal blood, testing of D negative donors for DEL by RHD PCR and high-throughput genotyping of blood donors. In addition, current limitations of the methods are illustrated.
